In order to apply for the Free or Reduced Hot Lunch program, please print and fill out this application and return to the main office. Please view the Income Guidelines (based on household side & income) Non Discrimination Statement as well.

Food Allergies and Special Diet Needs

If your child has a food allergy or a medical condition impacting their diet, please complete the Special Diet Statement, have it signed by your doctor, and return the form to the school office. addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a
written description of the alleged discriminatory action in sufficient detail to inform the Assistant
Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The
completed AD-3027 form or letter must be submitted to USDA by mail:
