In order to apply for the Free or Reduced Hot Lunch program, please print and fill out this application and return to the main office. Please view the Income Guidelines (based on household side & income) Non Discrimination Statement as well.
Food Allergies and Special Diet Needs
If your child has a food allergy or a medical condition impacting their diet, please complete the Special Diet Statement, have it signed by your doctor, and return the form to the school office.
